Once completed, please submit the reservation form via fax at (662)915-5410 or stop by our office in the Student Union, Room 419. Reservations are not confirmed until the applicant has received confirmation from a representative from the Department of Campus Programming.

**Reservations for rooms in the Student Union must be made through the UM Box Office in the Student Union or by calling (662)915-7411.
